- Photocopy of Child’s Aadhar card
- Photocopy of Birth Certificate
- Photocopy of Mother’s Aadhar
- Bona fide certificate
- Record sheet (academic)
- Transfer certificate (TC)
- Photocopy of Mother’s Bank Passbook 1st Page
- Photocopy of White Ration Card (if applicable)
- Latest passport size photographs (3)
